Blue Mountain Health System's case management department provides assistance to patients, residents and families in planning for post-hospital needs, coordinating community services, providing social and emotional support and communicating with insurance companies. The case management team includes nurses and social workers who work with physicians and therapists to ensure a smooth and comprehensive review of patient and resident needs. Case management services are available to hospital, home care, skilled nursing and recovery care residents and family planning clients.
