Hospitalist Program

What is a hospitalist?

Incare, P.C., is a dedicated group of internal medicine physicians who can provide you with care while you are staying at Community Medical Center. Hospitalists work closely with you primary care doctor, if you have one, to coordinate the best possible treatment plan for you. This exclusive inpatient focus allows hospitalists to devote more time, attention and energy to addressing your individual inpatient needs and concerns. This service is available 24 hours a day, 7 days a week, because we work right at the hospital.

How does it work?

Since the early 1990’s, a specialty called hospitalists has become a nationwide trend. Hospitalists are restricted to in-hospital care and range from complete management of the patient from admission through discharge, to medical consultations for other inpatients and/or

co-management of medical conditions for hospitalized surgical patients.

When a decision is made to admit you to the hospital, you may be seen by one of our hospitalist physicians and they will care for you while you are an inpatient. They have no office practice and you are returned to your primary care physician for continued care after discharge. If you do not have an established relationship with primary care physician, arrangements will be made for follow-up with a local physician, following your discharge from the hospital.

Hospitalists provide many benefits to you

Since hospitalists work in the hospital, they can see you more than once a day, if needed. Your hospitalists know every department in the hospital and can assist you through a smooth and speedy recovery process by following-up on tests and adjusting your treatment regimen throughout the day based on those test results.

Hospitalists are also nearby to answer your family’s questions. During critical and stressful moments, they provide honest and considerate answers to you and your loved ones. Since they work at the hospital, they are able to spend plenty of time talking with you and your family about your care.

How does the hospitalist know about my health history?

Hospitalists talk to your primary care physicians on a frequent basis. At the time of your admission to the hospital, your primary care physician calls the hospitalist and informs him of your condition. Your physician sends over records to provide as much information as possible about your illness. The process is similar to what your physician does when he or she refers you to a specialist for another opinion or additional tests and treatment.

During the course of your hospitalization, your hospitalist and primary care physician may talk further regarding your treatment. At discharge, your hospitalist will communicate with your primary care physician to discuss your further treatment needs, help arrange follow-ups, and prescribe the necessary medications. The hospitalist will send your hospital records to your primary care physician.

We provide benefits to your physician

Hospitalists help your primary care physician to be more available to you. Since a hospitalist will care for you during your hospital stay, your regular physician will be informed about your ongoing process and be ready to resume your care after your discharge.
