There is truly no place like home, especially if you are recovering from an illness. As an alternative to remaining in the hospital, you can recuperate in the comfortable surroundings of your own home.

Blue Mountain Home Health Care makes house calls to meet the health care needs of homebound clients in Carbon, eastern Schuylkill, Monroe, and upper Lehigh and Northampton counties.

Home health care is a medical and personal service provided to eligible patients in the comfortable and familiar surroundings of their own homes. To determine your eligibility, a member of the agency’s professional staff will schedule an initial evaluation visit and consult your physician to review your health care needs. Service is provided without regard to race, color, national origin, ancestry, age, gender or religion.

Home health care is an integral part of Blue Mountain Health System’s continuum of care. Services are available to patients who:

  • are under the regular care of a physician and the physician orders treatment;
  • are essentially homebound;
  • require intermittent skilled care;
  • will benefit from treatment in the home setting.

Registered nurses provide skilled care including:

  • Ostomy care and education
  • Behavioral health care
  • Catheter care
  • Diabetic teaching
  • Infusion therapy
  • Wound care and redressing
  • Medication monitoring/education
  • Skilled assessment and evaluation
  • Enteral feedings
  • Cardiopulmonary services
  • Health education

Medical social workers offer assistance with financial and psychosocial issues. They also provide counseling for patients and their families about the effects of an illness.
Physical therapists help patients regain their strength and muscle control through exercises designed to increase joint flexibility, coordination, and endurance.

Occupational therapists assist patients to regain their ability to perform independent activities of daily living such as bathing, dressing and cooking. The therapist can also asses your home for potential safety issues.

Speech therapists assist patients with speech and language impairments, feeding and swallowing difficulties, and cognitive impairment secondary to neurologic impairment.

Home health aides provide personal care for patients and assist nurses and therapists.

Blue Mountain Home Health Care is certified to provide care under Medicare, Medicaid, and Blue Cross plans. Most private insurances pay for home health care. A member of the Pennsylvania Association of Home Health Agencies, Blue Mountain Home Health Care is accredited by the Joint Commission on Accreditations of Healthcare Organizations and is licensed by the state.
