Unique Care at The Summit
Having to place a loved one in a nursing and rehabilitation center can be one of the most difficult, yet important decisions you may need to make. Residents admitted to The Summit at Blue Mountain Health System benefit from a unique model of care combining the security and resources of a hospital setting with the coordinated, interdisciplinary team approach of comprehensive care and rehabilitation. The Summit is conveniently located on the Gnaden Huetten campus of the Blue Mountain Health System allowing prompt access to specialists, diagnostic services and emergency care, if needed.
At The Summit, long-term and short-term rehabilitation care is provided. Our goal-oriented, interdisciplinary team works closely with each resident to reach quality outcomes as they progress through the continuum of care. Offering rehabilitation services close to home enables family and friends to visit frequently and provide much needed support during the recovery process. Rehabilitation services are offered, but not limited to residents who have experienced:
- total joint replacement;
- or are in need of IV antibiotic therapy.
To facilitate a timely transition back to the home or residence, our social workers work closely with the resident and family to ensure that community resources are offered and arranged prior to discharge.
At The Summit, residents have the optional conveniences of laundry services, hair care services, cable television and private telephones. Cell phones are also permitted. Our residents have the freedom and privacy to attend to their personal needs – from managing their own financial affairs, if they are able, to decorating their rooms with personal belongings. They are also able to participate in the planning of their treatment and are assured of the confidentiality of their medical information.
Recreational and Therapeutic Activities
Activities at The Summit allow residents to develop friendships and rapport with their peers and staff, forming relationships that frequently extend beyond admission. Our activities department staff design entertaining and stimulation programs, with input from residents, on a monthly basis.These activities not only provide enjoyment for our residents, but they also assist residents in meeting specific goals in their recovery process. Residents are also encouraged to participate in the resident council meetings, while family members are encouraged to participate in the family council. Both groups periodically share their ideas and suggestions for improvement with facility staff.
Our nutritional services department prepares appetizing and nutritious meals for our residents. Hostesses are available to meet with residents daily to determine their personal meal preferences and assist them with adhering to any special dietary needs. Residents are encouraged to enjoy their meals in the pleasant surroundings of our dining room or they may dine in the comfort of their own room.
The Summit welcomes referrals from physicians, discharge planners, clinical specialists, social workers, family members, insurance representatives and case managers. Our staff will gladly arrange for a guided tour of our facility.
