The Community Health Needs Assessmment is a requirement of the Internal Revenue Service of all nonprofit hospitals. This is the second CHNA required by hospitals and it is to be updated every three years. It was expected of hospitals not duplicate the work of other agencies that routinely complete Community Health Needs Assessment surveys. The Blue Mountain Health System enlisted the help of the Eastcentral Pennsylvania Area Health Education Center with the research and analysis of the information gathered. We thank them for their dedication and assistance with this document.
This assessment includes a demographic overview of the community served, a review of process and methods used, a summary of input from other agencies, a description of health needs, a review of the leading causes of death and health concerns, a review of the health system’s programs and services, and implementation strategy.
The Community Health Needs Assessment was adopted by the Blue Mountain Health System Board of Trustees on August 3, 2016.
